Customer Request Form

A 'customer request' is a request for service and/or action by Council and can be submitted by completing the online form below or by clicking here (DOC, 185KB) to download a hard copy of the form.

Printed forms can be submitted to council by:

  • Posting to Murrindindi Shire Council, PO Box 138, Alexandra, 3714
  • Faxing to (03) 5772 2291
  • In person to any Council office at Alexandra, Kinglake or Yea
  • Email to customer@murrindindi.vic.gov.au
